Documents Needed to Enroll

• Birth Certificate or Passport

• Child’s current Health Insurance Card

• Immunization Records

• Child’s most recent Report Card or if coming into 1st Grade, provide their most recent Progress Report

• A copy of the child’s TB test results (TB test must be taken in the U.S.).
